Scrotoplasty Photographs
Scrotoplasty is performed by making rotational advancement flaps with majora tissue. Please note that intraoperative photos are included and may be uncomfortable to view for some. For patients eventually in need of the inflatable penile implant and testis implants, it is especially important to have a pouch-like anatomically positioned scrotum to hold the pump component of the inflatable implant and the testicle.

Scrotums can sometimes have indentations; this is managed at time of testis implant insertion, which reduces or gets rid of the anterior dimpling.
Testis implants are placed via an anterior scrotal incision on each side of the scrotum. This approach allows anterior and dependent implant positioning, minimizing the risk of the implant moving or settling at a higher position.
6 months postop. Anatomically positioned anteriorly and superiorly, with good distance away from the anus.
About 4 months after testis implant insertion.

Immediate postop view; tube at bottom of scrotum is a drain that stays for about 5 days. Scrotums are larger after phalloplasty (compared to after metoidioplasty). There is more majora tissue for creation of a scrotum in phalloplasty than in metoidioplasty.

Phallus lifted to better reveal the pouch-like scrotum. Photo taken on postop day # 1. Depending on the amount of swelling and anatomy, midline clefts can be seen as the tissue swells around the suture lines.
2 days postoperative after RF phallo and scrotoplasty; drain remains in the scrotum for 5 days

About 1 year postop
Patient A: 2 weeks postop after testicular implant insertion
Patient A: 4 weeks postop
Patient A: after 18 months, the scrotal skin stretched, which accommodated testis implants that were larger than the original testis implants placed about 1 year prior
Immediate postop photo. Scrotums in metoidioplasty are smaller than in phalloplasty by design.
Immediate postop photo after metoidioplasty with scrotoplasty. Note that the scrotum is smaller in meta than in phallo.
