For patients who turn 65 years old or who are 65 years old and older, Medicare generally takes over as the primary insurance.
What this Means for Our Patients

You are responsible for all costs. By choosing our practice, you agree to be financially responsible for the full cost of all services, procedures, and treatments. This includes office visits, surgeries, and all other care that would otherwise be covered by Medicare Part B.

No claims will be submitted to Medicare as we are not allowed to. We will not submit claims to Medicare on your behalf. Likewise, you or your secondary insurance provider (including Medigap) cannot submit a claim to Medicare for any services from our practice.

This includes Medicare Advantage Plans. Your Medicare Advantage plan will not cover services at our practice.

Exception: in the event you also have insurance coverage through an employer with more than 20 employees, we will attempt to work with the insurance plan elected by the employer.
Your Right to Choose:
We want you to be fully informed and confident in your healthcare decisions. You have the right to receive Medicare-covered services from physicians and practitioners who accept Medicare. This notice is intended to ensure you understand your options.
