Penis Reconstruction

For some patients, especially ALT phalloplasty patients, urethral reconstruction and penis reconstruction are performed at the same time to lengthen the urethra, repair a fistula/stricture, and reduce the girth of the penis. Glansplasty can sometimes be performed at the same time.

For other patients, penis shortening or other reconstructive procedures are required. The following photos show various patients with before and after photos of their reconstruction.
Patient did not have UL at time of ALT phalloplasty due to thick thigh. The urethra was rolled 6 months later and he developed a base of penis fistula and the tip of the urethra is not at the tip of the penis.
Urethral fistula repaired, and penis girth decreased.
Top view with glansplasty.
Patient did not get UL at time of ALT phalloplasty. The urethra was rolled about 6 months later but the tip could not be rolled. He therefore urinated out of a urethra in the middle of his penis.
The urethra was extended closer to the tip and the penis girth was reduced. Glansplasty was also performed.

Glansplasty was performed at time of urethral reconstruction and penis girth reduction. Testicular implants were also inserted at the same surgery.
Immediate preoperative photo. The patient is over one year postop from phalloplasty and has healed well. He wanted length reduction. The blue marks indicate areas where the phallus would be cut and shortened, as well as where the glansplasty incisions will be made.
Immediate postoperative photo. With removal of the old glans, a new glans is created.
2 weeks postop. There is scabbing along the suture line and the scabs will slough.
